Qualifications

• Current and valid Massachusetts LPN license.
• Experience with practical nursing and medication administration.
• Strong documentation and communication skills.
• Ability to work within the LPN scope of practice.
• Ability to stand and stay active throughout the shift.

Responsibilities

• Administer medications and treatments per orders and community procedures.
• Monitor residents for changes in condition and daily functioning.
• Provide direct nursing care within MA LPN scope of practice.
• Document health information accurately and promptly.
• Participate in assessments and care-plan reviews with nursing leadership.
• Coordinate refills and communicate with pharmacies and providers.
• Support admissions, transitions, and changes in level of care.
