Job summary

All American Assisted Living at Raynham is seeking an LPN to join our Wellness team on a per diem basis. You will provide personal care, supervision, and companionship to residents, supporting independence and dignity while following individualized service plans.

Responsibilities include assisting with ADLs, mobility, meals, documentation, and reporting changes to nursing staff. Massachusetts training requirements apply, with a focus on safety, patient rights, and compassionate care.

Qualifications

• Must complete Massachusetts assisted living personal-care training.
• Must complete community orientation and competency reviews.
• Must demonstrate competency in the personal-care tasks assigned.
• Must receive appropriate training before assisting with self-administered medication management.

Responsibilities

• Provide respectful personal care and assistance with activities of daily living.
• Assist residents with bathing, dressing, grooming, toileting and hygiene.
• Support mobility, transfers, and positioning per care plans.
• Assist with meals and dining needs; offer companionship and reassurance.
• Document care accurately and promptly; report changes to nursing staff.
• Maintain clean, safe living areas and respect resident privacy.
