Job summary

Atlantic Health System invites a Per diem Social Worker to join the Homecare & Hospice division in Morristown, NJ. You will conduct home visits, provide disease counseling, and lead end-of-life discussions aligned with goals of care.

You will perform psychosocial assessments, identify safety risks, and coordinate with an interdisciplinary team to develop and implement care plans. Driving to community resources is required.

Qualifications

• Master's in Social Work from an accredited school required.
• Licensed in NJ (LSW).
• State of NJ LCSW preferred.
• 1–2 years' social work experience preferred.
• Expertise in community resources preferred.
• Excellent customer service and communication skills.
• Personal vehicle required for home visits; valid driver's license.

Responsibilities

• Provide home visits with disease counseling and end-of-life discussions.
• Conduct comprehensive psychosocial assessments.
• Identify safety risks and available support systems for patients and families.
• Collaborate with interdisciplinary team to develop care plans.
• Counsel patients and families on coping strategies for disease.
• Maintain documentation according to agency guidelines.
