Atlantic Health System is seeking a per diem Social Worker for the Atlantic Homecare & Hospice division in Morristown, NJ. The role involves home visits to patients, disease counseling, and discussions about goals of care, with referrals to community resources as needed.

Responsibilities include psychosocial assessments, interdisciplinary collaboration to develop and implement the plan of care, counseling for patients and families, and ongoing evaluation of social work services.

Responsabilidades

• Home visits with home care patients, providing disease counseling and end of life discussions with goals of care, community resource referrals, assistance with applications to community resources.
• Performs comprehensive psychosocial assessments and identifies safety concerns for patients and families.
• Collaborates with the interdisciplinary team to develop and implement the plan of care, including short- and long-term goals.
• Provides counseling to patients and families to cope with disease processes and participates in family conferences.
• Evaluates the effectiveness of social work services and maintains documentation per agency guidelines.
• Participates in Performance Improvement activities and helps develop/revise job functions and policies.
• Assists with growing agency business through community education and outreach and accepts other assignments as needed.
