Job summary

RWJBarnabas Health's Trinitas Regional Medical Center in Elizabeth, NJ seeks a Per Diem Social Worker to provide frontline psychosocial support across the care continuum. You will collaborate with physicians, nurses and the case-management team to address social, financial and emotional needs and support discharge planning.

The role requires an MSW, a current NJ LSW or LCSW in good standing, and preferably hospital-based social work or case management experience.

Qualifications

• Master's Degree in Social Work (MSW) from an accredited program.
• Active NJ LSW or LCSW license in good standing.
• Hospital-based social work or case management experience preferred.
• Bilingual Spanish highly desirable.

Responsibilities

• Identify, screen, and complete psychosocial assessments to guide interventions.
• Provide direct counseling to patients and families navigating illness and care transitions.
• Formulate discharge plans and link patients with community resources.
• Collaborate with interdisciplinary team during case reviews to address social needs.
• Coordinate liaison with palliative care, hospice, home care and long-term facilities.
• Document care plans and statistics in the EMR accurately and timely.
• Support staff education and mentorship for trainees when available.
