Qualifications

• Graduate of an accredited school of Social Work (MSW) required.
• Previous medical hospital social work required.
• NJ LSW or LCSW required.

Responsabilités

• Assesses the patient’s psychosocial situation and plans interventions and goals to improve patient/family functioning.
• Participate in Multidisciplinary Rounds: Psycho/Social Context.
• Advocate for patient and family discussions involving Advanced Care Planning.
• Other duties as assigned.
