Job summary

Serenity Hospice Care in New Jersey seeks a dedicated Social Worker to join our Monmouth County team. This full-time role runs Monday–Friday 8:30am–4:30pm, focusing on psychosocial assessments, bereavement services, and interdisciplinary collaboration.

The ideal candidate holds an MSW with plans to obtain LSW within 6 months, is proficient in Microsoft Office, and can work well independently and with teams. A valid driver’s license is required.

Qualifications

• MSW required, must obtain LSW within 6 months of employement.
• Excellent computer skills, especially Microsoft Office.
• Flexible and able to adapt to changes as needed.
• Able to self-manage and work with teams
• Organized & Detail Orientated.
• Reliable and trustworthy.
• Drivers License with reliable transportation.

Responsibilities

• Assist with performing the social services section of the assessment process as well as provide on-going care to patients and their families as needed.
• Assist with performing bereavement services (calls, mailings, documentation).
• Provide updates to the bereavement plan of care as necessary.
• Attend Interdisciplinary Team meetings as needed.
• Managing and completing all psych-social assessments
• Educating the patient and family about services
• Performs other duties as assigned.
