Job summary

Talentify is seeking a PRN Case Manager for Select Specialty Hospital LTACH in Phoenix, focusing on discharge planning, utilization reviews, and care coordination. You will report to the Director of Case Management and provide social work services as needed per state guidelines.

The role requires current licensure in a clinical discipline or social work and previous inpatient discharge planning experience. Availability for a minimum of 2 shifts per month on weekdays is expected.

Qualifications

• Current licensure in a clinical discipline per state guidelines (RN/LPN/LVN or RT) or Social Work license.
• Inpatient hospital experience in critical/acute care (ICU/step-down/ventilator care).
• Experience in acute medical case management and planning for chronically critically ill patients.

Responsibilities

• Develops patient-specific, safe discharge plans.
• Performs utilization criteria reviews.
• Coordinates with payor sources to assure proper reimbursement.
• Ensures compliance with Utilization Management policies.
• Coordinates UR with Quality Management for licensure/accreditation.
• Manages fiscal responsibilities and maximizes insurance coverage.
• Leads multi-disciplinary team meetings with physicians, nurses, RTs and rehab therapists.
