Job summary

Select Specialty Hospital, a part of Select Medical, seeks a PRN Case Manager at our LTACH to coordinate discharge planning, utilization reviews and social work services. You will report to the Director of Case Management and support ICU/critical care patients with a patient-centered care plan.

Qualified candidates will hold licensure as RN/LPN/LVN or RT or a Social Work license, with prior inpatient case management and discharge planning experience preferred.

Qualifications

• Current licensure in a clinical discipline RN/LPN/LVN or Respiratory Therapist or Social Work license per state guidelines.
• RN/LPN/RT/SW/CM inpatient hospital experience in acute/critical care.
• Experience in acute medical case management and directing a plan of care for chronically critically ill populations.

Responsibilities

• Develops and implements a patient-specific, safe and timely discharge plan.
• Performs verification of utilization criteria reviews.
• Builds relationships with payor sources to assure proper reimbursement for services.
• Demonstrates compliance with Utilization Management policies and procedures.
• Coordinates UR compliance with Quality Management.
• Maintains fiscal responsibilities and negotiates reimbursement within the plan of care.
• Facilitates multi-disciplinary team meetings.
