Job summary

Select Specialty Hospital is seeking a PRN Case Manager to join our critical illness recovery hospital team. The role emphasizes discharge planning, utilization review, and care coordination for chronically critically ill patients in an inpatient setting.

Qualified candidates will hold an active clinical license (RN/LPN/LV Thoracic Respiratory Therapist or Social Work SW/MSW) per state guidelines and have prior discharge planning experience.

Qualifications

• Requires current licensure in a clinical discipline (RN/LPN/LV N), Respiratory Therapist, or Social Work SW/MSW per state guidelines.
• Minimum inpatient hospital experience in case management or discharge planning preferred.
• Strong communication and collaboration with multidisciplinary team.

Responsibilities

• Develops and implements patient-specific discharge plans.
• Performs utilization and resource reviews.
• Coordinates with payors to maximize appropriate reimbursement.
• Ensures documentation and compliance with case management policies.
• Leads multidisciplinary team meetings and care coordination.
• Reports to the Director of Case Management.
