Job summary

Select Medical's Select Specialty Hospital LTACH in Atlanta seeks a PRN Case Manager. You will handle discharge planning, utilization reviews, and social work services per state guidelines, reporting to the Director of Case Management.

Requirements include current licensure in nursing, RT, or SW per state guidelines; prior discharge planning and inpatient care experience are preferred. Shifts are as needed, typically weekdays, 8a-5p.

Qualifications

• Current licensure in a clinical discipline (RN/LPN/LV N) or Respiratory Therapist or Social Work SW/MSW.
• Previous discharge planning experience preferred.
• Experience in inpatient hospital/critical care settings.

Responsibilities

• Render utilization reviews and resource management for discharge planning and treatment plans.
• Perform medical record documentation and report to Director of Case Management.
• Coordinate with payors to maximize appropriate reimbursement and ensure quality care.
• Lead multi-disciplinary meetings with physicians, nurses, RTs, and rehab staff.
