Job summary

Select Specialty Hospital is seeking a Case Manager for a full-time, Mon-Fri 8-5 role at our LTACH. Requires licensure in a clinical discipline (RN/LPN/LVN or RT) or Social Work per state guidelines; inpatient discharge planning experience preferred.

You will lead utilization reviews, discharge planning, treatment plan and financial management, document medical records, and collaborate with the care team to ensure timely, patient-centered outcomes.

Qualifications

• Current licensure in a clinical discipline (RN/LPN/LVN) or RT or Social Work license per state guidelines.
• Inpatient hospital experience in critical/acute care; RN preferred.
• Care management and discharge planning experience preferred.

Responsibilities

• Develops patient-specific, safe and timely discharge plans.
• Performs verification of utilization criteria reviews.
• Coordinate with payors to maximize reimbursement and plan care.
• Demonstrates compliance with Utilization Management policies.
• Facilitates multi-disciplinary team meetings with physicians, nurses, RTs and rehab staff.
