Job summary

Select Specialty Hospital is a Critical Illness Recovery Hospital (LTACH) in need of a Case Manager to oversee discharge planning, utilization reviews and medical record documentation. This full-time, Monday-Friday 8-5 role reports to the Director of Case Management and supports social work services per state guidelines.

The ideal candidate holds current licensure in nursing (RN/LPN/LVN) or respiratory therapy or social work, with inpatient discharge planning experience.

Qualifications

• Current licensure in a clinical discipline (RN/LPN/LVN) or Respiratory Therapist (RT) OR Social Work license per state guidelines.
• Experience in inpatient discharge planning or case management preferred.

Responsibilities

• Develops patient-specific, safe, timely discharge plans.
• Performs utilization criteria reviews and coordinates with payors for reimbursement.
• Maintains medical record documentation and reports to the Director of Case Management.
• Coordinates with multi-disciplinary teams including physicians, nurses, RTs and therapy staff.
• Manages financial aspects of discharge planning and resource use to maximize insurance coverage.
