Job summary

Select Specialty Hospital in Phoenix is seeking a Case Manager (PRN) with licensure in nursing, respiratory therapy, or social work to support our Critical Illness Recovery LTACH. The role emphasizes patient-focused care and coordination within a high-acuity setting.

The Case Manager will handle utilization reviews, discharge planning, treatment plan management, and financial oversight, while collaborating with physicians, nurses, and therapists to ensure timely, high-quality patient care.

Qualifications

• Current licensure in a clinical discipline (RN/LPN/LVN) or Respiratory Therapist OR Social Work license per state guidelines.
• Previous RN/LPN/RT/SW/CM inpatient experience in critical care or acute care settings.
• Experience in acute medical case management and planning for chronically critically ill populations.

Responsibilities

• Develops patient-specific, safe discharge plans and coordinates with payors for reimbursement.
• Performs utilization reviews and manages case treatment plans and finances.
• Completes medical record documentation and leads multi-disciplinary team meetings.
