Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in the Lansing, MI area. The nurse will provide direct patient care in home or alternate sites, perform assessments, care planning, education, and ensure safe IV therapy per care matrices and standards.

Requires Michigan RN license; BSN preferred; on-call rotation; ability to travel up to 75 miles; expected to provide compassionate, high-quality care in patients' homes.

Qualifications

• Current RN license in Michigan in good standing.
• RN degree; BSN preferred.
• Advanced certifications in infusion (CRNI/ONS/IgRN) preferred.
• On-call rotation required; nights/weekends possible.
• Administrative oversight and regulatory compliance responsibilities.
• Emergency availability for after-hours.
• Minimum 3 years infusion nursing experience recommended.
• ICU/Cardiac care experience preferred.

Responsibilities

• Provide direct care for assigned patients, coordinating with care team.
• Educate patients and care partners on home infusion therapy.
• Assess home environments for safety and appropriateness.
• Develop and evaluate the plan of care with the team.
• Manage schedules via company platform; communicate changes.
• Coordinate patient care using approved systems.
• Participate in nurse on-call rotation.
• Document encounters and care activities per policy.
• Maintain knowledge of Soleo therapies and protocols.
• Ensure POTs and clinical documents are submitted timely.
