Job summary

Chapters Health System is seeking a Per Diem RN to assess patients, coordinate Plan of Care with the IDG, and provide palliative and supportive care. The role requires a current RN license, BLS, and clinical experience in hospice or hospital settings, with EMR/EHR proficiency and a valid driver's license.

The position emphasizes compassionate, patient-centered care with flexibility in schedule and the ability to work across multiple facilities within the service area.

Qualifications

• Current RN license in the state of employment.
• Active BLS for Healthcare Professionals (AHA or Red Cross).
• Minimum one year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance.
• Reliable transportation to meet visit schedule.
• Ability to communicate in English (verbal and written).
• Flexible schedule to meet Hospice House patient needs.
• Ability to assist with activities of daily living and navigate facilities.
• If assigned to PACE, Alzheimer's Disease and Related Dementias training may be required.

Responsibilities

• Explain services to patients/families and address questions empathetically.
• Identify admission objections and provide responses.
• Educate patients/families on admission requirements and needs.
• Complete informed consent and initial assessment per policy.
• Provide information to physicians and initiate Plan of Care.
• Initiate skilled nursing interventions to relieve symptoms and discomfort.
• Obtain physician orders and complete documentation.
• Represent the company at facilities and coordinate referrals.
• Communicate with Admissions for timely follow-up and handoffs.
