Job summary

The Per Diem RN at Chapters Health System provides compassionate, patient-centered care within the Interdisciplinary Group (IDG), coordinating the Plan of Care and delivering palliative support to patients and families.

Responsibilities include assessing needs, utilizing nursing processes, and ensuring effective communication with IDG while maintaining quality care in hospice settings. This role requires flexibility and strong English communication skills.

Qualifications

• Current RN license in Florida and in good standing.
• Active BLS for healthcare professionals.
• Minimum 1 year hospice or hospital nursing experience preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provides and manages direct care to patients and families as part of IDG.
• Coordinates the Plan of Care with the IDG.
• Educates patient and family on disease, palliative interventions, and safety.
• Reports changes in patient condition to IDG or other professionals.
• Performs other duties as assigned.
