Job summary

Sanctuary Hospice is seeking a Per Diem On-Call Registered Nurse (RN) to join our team, delivering compassionate hospice care to patients and supporting their families during critical moments. As an On-Call RN, you will identify patient and family needs, coordinate care, and provide preventive, compassionate support in home settings.

Requirements include an active RN license, minimum 1 year in medical-surgical/acute care (hospice a plus), CPR if required, and proficiency with EMR systems.

Qualifications

• Bachelor's degree in nursing or health care preferred.
• Hold current unencumbered license as a Registered Nurse (RN).
• Minimum one year RN experience in medical-surgical/acute care; hospice experience a plus.
• Must be computer proficient in typing and various programs, including EMR background.
• Possess and maintain CPR certification if required by state.

Responsibilities

• Respond to a variety of patient needs, from urgent to routine, in the home setting.
• Provide empathetic assessments of physical, emotional, social, and spiritual needs.
• Maintain accurate documentation compliant with agency and regulatory standards.
• Support families by sharing expertise and guidance during a challenging time.
