Job summary

Oasis Hospice provides compassionate, team-based end-of-life care in the home setting. The RN delivers hospice nursing services, supports families, and collaborates with the IDG to plan and implement patient care. This per diem, part-time position tracks visits with a tiered pay scale and requires a current California RN license.

The role emphasizes independent work, crisis response, and adherence to HIPAA while maintaining high-quality patient care and family support.

Qualifications

• Graduate from an accredited school of nursing.
• Ability to work independently and make quick judgments.
• Ability to supervise others appropriately.
• Ability to respond to crises outside of a hospital setting.

Responsibilities

• Regularly assess and reassess patient nursing needs.
• Provide Hospice nursing services, treatments and preventive procedures.
• Develop and re-evaluate patient/family care plans with the IDG.
• Attend team conferences and maintain patient records.
• Coordinate care for patients in SNF, NF, ICF or MR.
