Job summary

Hospice of the West is seeking a Full Time On-Call Registered Nurse (RN) to join our team, providing exceptional care and support to hospice patients and their families during their most critical moments. As an On-Call RN, you will coordinate care, identify patient and family needs, and offer preventative measures and compassionate support.

Responsibilities include responding to patient needs from urgent to routine, conducting comprehensive assessments, maintaining documentation, and supporting

Qualifications

Bachelor's degree in nursing or health care preferred.
Hold current unencumbered RN license.
Minimum 1 year RN experience in medical‑surgical/acute care; hospice experience a plus.
Computer proficiency with EMR and related programs.

Responsibilities

Be the Lifeline: respond to a variety of patient needs with compassionate support.
Provide empathetic assessments across physical, emotional, social and spiritual needs.
Maintain accurate documentation in patient home and agency records.
Support families by sharing expertise and guidance during end‑of‑life care.
