Job summary

Agape Hospice Care is seeking an On-Call Registered Nurse (RN) to join our weekend hospice care team. The RN will provide skilled nursing support in patients' homes, coordinating with the interdisciplinary team, families, and referring agencies.

The position requires an RN license, CPR certification, and a strong commitment to compassionate patient care in a fast-paced weekend on-call environment.

Qualifications

Graduate of an accredited nursing program with RN licensure.
Current state RN license and driver's license.
Two years' RN experience in a clinical setting; hospice/home health preferred.
Current CPR certification.
Proficient computer skills; MS Office experience preferred.

Responsibilities

Administer skilled nursing care for clients in their place of residence.
Coordinate care with the interdisciplinary team, patient, family, and referring agency.
Coordinate of care and collaborate with other healthcare professionals.
