Job summary

Heart of Hospice, a part of LHC Group and member of the Optum family, seeks a Per Diem RN Case Manager to manage Plan of Care for hospice patients. You will evaluate performance, ensure policy adherence, and contribute to high-quality patient care.

Responsibilities include initial hospice eligibility assessment, ongoing family-patient support, and collaboration with care teams to deliver daily, compassionate care. LA-specific RN experience required and CPR preferred.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• LA state-specific requirement: two years RN experience, with substitution rules for hospice experience as described.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Reassesses patient needs at least every 15 days and adjusts the plan of care accordingly.
• Initiates and coordinates the plan of care with accurate clinical notes.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician, and IDG members to deliver daily care.
