Job summary

Heart of Hospice, part of UnitedHealth Group, is seeking a Per Diem RN Case Manager to lead the delivery of the Plan of Care for hospice patients. The role requires evaluating care quality, coordinating between families and care teams, and maintaining detailed clinical documentation.

Responsibilities include rapid eligibility assessment, ongoing reassessments, plan-of-care initiation, and collaboration with physicians and IDG members. Louisiana-specific requirements apply.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• LA: a minimum two years full time experience working as an RN or two years as an RN substitute allowed.
• Current CPR Certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs; reassesses at least every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes.
• Documents problems, goals, interventions, and patient/family responses.
• Collaborates with patient/family, attending physician and IDG for daily care.
