Job summary

Heart of Hospice, part of the LHC Group and Optum family, is seeking a Per Diem Registered Nurse Case Manager to deliver hospice care at home. You will evaluate care plans, coordinate nursing services, and ensure quality patient outcomes in coordination with the care team.

Responsibilities include initial eligibility assessments, continuous performance reviews, and documentation of patient progress. A valid RN license in the state and a driver’s license are required, with competitive hourly pay

Qualifications

• Current unrestricted RN licensure in the state of practice; required.
• 1+ years of clinical experience; required.
• Current driver's license, valid vehicle insurance, and access to dependable transportation or public transit; required.

Responsibilities

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify patient/family needs and reassess every 15 days.
• Initiate and coordinate the plan of care with accurate clinical notes.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with patient/family, attending physician and IDG to provide daily care.
