Job summary

Louisiana Hospice and Palliative Care, part of LHC Group, a UnitedHealth Group company, is hiring a Per Diem Registered Nurse Case Manager. You will oversee the Plan of Care for hospice patients, perform initial evaluations within 48 hours of referral, and coordinate care with the IDG to ensure quality, compassionate support.

Qualifications include current unrestricted RN licensure in LA, 1+ year of clinical experience, a valid driver's license, and CPR certification preferred.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license with access to a dependable vehicle or reliable public transit.
• State-specific requirement: LA requires two years full-time RN experience; two years hospice LPN can substitute for RN.
• Preferred: CPR certification (or ability to obtain within 90 days).

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies the patient's needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes.
