Job summary

Elite Hospice in Forrest City, AR, part of the LHC Group family within UnitedHealth Group, is seeking a Per Diem RN Case Manager to deliver and evaluate the Plan of Care for hospice patients in the local area.

You will assess eligibility quickly, coordinate care with the IDG, and document patient progress while supporting families with compassionate care. This role emphasizes quality, compliance, and collaborative teamwork in a per diem capacity.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• Preferred CPR Certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within 48 hours of referral.
• Identifies patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes.
• Documents problems, goals, interventions, and patient/family responses.
• Collaborates with patient/family, attending physician and IDG daily.
