Job summary

UnitedHealth Group's Deaconess Hospice in Biloxi, MS is seeking a Per Diem Registered Nurse Case Manager to deliver the Plan of Care for hospice patients and ensure high-quality care. You will evaluate performance, maintain documentation, and support the care team in a fast-paced, patient-centered environment.

The role requires MS RN licensure, 1+ year clinical experience, and a valid driver's license. It offers per diem scheduling with competitive hourly pay and a comprehensive benefits package

Qualifications

• Current unrestricted RN licensure in the state of practice (Mississippi).
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Make the initial nursing evaluation within 48 hours of referral.
• Identify patient/family needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care with accurate clinical notes.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with patient/family, attending physician and IDG members daily.
