Job summary

Crescent Hospice is seeking a Per Diem Hospice RN Case Manager to join our nursing team delivering hands‑on hospice care, coordinating services, and guiding patients and families through the hospice journey across York, Chester, and Lancaster County.

As an RN Case Manager you’ll be part of an interdisciplinary team, focused on compassionate, clinically excellent care grounded in dignity, peace, and human connection.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver’s license, vehicle insurance, and access to dependable transportation.
