Qualifications

• Current Texas RN license.
• Compassionate, dependable, and organized.
• Strong clinical assessment and communication skills.

• Hospice or home health experience preferred.

Responsibilities

• Manage a full patient caseload with timely coordination of care.
• Perform skilled nursing visits and ongoing patient assessments.
• Develop and update individualized plans of care.
• Provide education, guidance, and support to patients and families.
• Collaborate with physicians and the interdisciplinary hospice team.
• Maintain timely and accurate clinical documentation.
• Participate in IDT meetings and on-call rotation as assigned.
• Coordinate changes in patient condition and ensure continuity of care.
