Job summary

At Home Hospice in Beaumont, TX is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients. You will conduct initial evaluations, collaborate with the care team, and implement care plans addressing physical, psychosocial, emotional, and spiritual needs.

The role requires current Texas RN licensure, 1+ year of clinical experience, and the ability to travel within a 50-mile radius. Home-based hospice experience is preferred and shifts are PRN.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies patient/family needs and re-assesses as needed, at least every 15 days.
• Initiates and coordinates the plan of care with the IDG.
• Documents problems, goals, interventions and patient/family responses.
• Collaborates with patient/family, attending physician and other IDG members.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes on patient condition.
• Do not discriminate and provide care in diverse home situations.
