Job summary

Altus Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community. Schedule: Monday - Friday, 8 am - 4:30 pm, On-call rotation required.

Territory: Williamson and surrounding counties. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey as part of our interdisciplinary care team.

Qualifications

Hold current unencumbered RN license.
Minimum one year RN experience in medical-surgical/acute care; hospice experience is a plus.
Must be computer proficient, including EMR usage.
CPR certification required if state regulations apply.

Responsibilities

Empower Through Engagement: respond to clinical referrals with care and expertise.
Collaborative Care Champion: work with Clinical Director to assess eligibility for hospice services.
Nurturing Direct Care: deliver hands-on hospice nursing care and coordinate services.
Nursing with Heart: provide top-notch care rooted in nursing principles.
Educator and Advocate: educate patients and families to participate in care plans.
Call of Compassion: share call duties and support the team when needed.
