Altus Hospice Care is seeking a Per Diem Hospice Registered Nurse Case Manager to join our caring team in Marble Falls and surrounding communities. You will deliver hands-on hospice nursing care, coordinate services, and support patients and families through every step of their journey.
As an RN Case Manager, you will collaborate with the Clinical Director to assess eligibility, ensure quality care, and educate families on care plans.
Competenze
- Hold active RN license without restrictions.
- Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
- Proficient in computer use and EMR systems; CPR certification may be required by state.
Mansioni
- Deliver hands-on hospice nursing care and coordinate services.
- Guide patients and families through the hospice journey with compassion and clinical excellence.
- Collaborate with Clinical Director to assess patient eligibility for hospice services.
- Provide nursing care rooted in established principles to ease pain and maintain comfort.
- Educate patients and families to participate in personalized care plans.
