Job summary

Altus Hospice is seeking a compassionate Per Diem Hospice RN Case Manager to join our nursing team in Kingwood, TX. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through the hospice journey with empathy and clinical excellence.

As part of our interdisciplinary team, you’ll educate families, participate in on-call duties, and support a smooth transition into care plans. We value dignity, peace, and human connection in every life we touch.

Qualifications

Must hold current unencumbered RN license.
Minimum of 1 year RN experience in medical-surgical/acute care; hospice experience is a plus.
Proficient with computers and EMR systems.
CPR certification if required by state.
Valid driver's license and reliable transportation.

Responsibilities

Empower through engagement by handling clinical referrals with care and expertise.
Collaborate with Clinical Director to assess eligibility for hospice services.
Provide direct care and ensure patient comfort and quality of life.
Apply nursing skills to deliver compassionate, evidence-based care.
Educate patients and families to create personalized care plans.
Participate in on-call duties as part of the team.
