Constellation Health Services seeks a Nurse Practitioner with Hospice/Palliative focus for PRN/home visits in Derry, Bradford and Nashua, NH. Must hold a DEA license and hospice experience is required.

Rates include $50/hour for office/meeting time, $115/visit for Telehealth, and $175/visit for face-to-face visits. The NP will work under a collaborating physician to evaluate patients, plan care, prescribe as needed, and coordinate with the hospice team.

Formación

• Current NP license and DEA license
• CPR certification
• Home health/hospice experience preferred
• Excellent communication and documentation skills
• Ability to work independently and in interdisciplinary teams

Responsabilidades

• Provide comprehensive hospice/palliative care to patients in home settings under physician supervision
• Perform history, physical examinations, and assess patient health status
• Develop and implement individualized care plans and manage medications
• Conduct hospice face-to-face visits as required by program guidelines
• Coordinate with hospice interdisciplinary team and family caregivers to ensure smooth care
• Educate patients and families on disease process, symptom management, and end-of-life care
• Maintain accurate clinical documentation and ensure compliance with HIPAA and state regulations
