Job summary

Constellation Health Services is seeking an experienced Nurse Practitioner focused on Hospice/Palliative Care for PRN/per diem work in Waterville, ME. DEA license and hospice experience are required.

You will provide comprehensive hospice services under physician supervision, evaluating health history, diagnosing conditions, and directing plan of care in patients' homes. You will collaborate with the hospice interdisciplinary team, conduct face-to-face visits, and ensure quality patient care

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current DEA license and nursing licensure in state with CPR certification.
• Licensure, education, training and qualifications required by state law to practice.
• Accreditation from an approved certifying body for APN as required by state.

Responsibilities

• Provide medical care and manage terminal illness under physician supervision.
• Perform history and physical examinations and document findings in the medical record.
• Develop individualized care plans, prescribe medications, and coordinate hospice care services.
• Complete hospice face-to-face visits to assess eligibility for hospice periods.
• Communicate with hospice medical director regarding eligibility and recertification.
• Coordinate care with interdisciplinary team and other healthcare practitioners.
• Educate patients and families on preventive care and end-of-life issues.
• Maintain HIPAA-compliant documentation and participate in quality improvement initiatives.
