Constellation Health Services is seeking a Nurse Practitioner with Hospice/Palliative Care expertise for PRN/Per Diem assignments in Brockton and East Bridgewater, MA. The NP provides comprehensive hospice assessments and participates in care planning under physician supervision.
The ideal candidate has Hospice/Palliative experience, holds a valid MA NP license, DEA license, CPR certification, and is comfortable working in home-based settings with interdisciplinary teams to optimize patient
Competenze
- Hospice/palliative care experience required.
- DEA license is required.
- Current MA nursing license and CPR certification.
- Previous home care/hospice experience preferred.
- Certified in Hospice and Palliative Nursing (CHPN) preferred.
Mansioni
- Provide medical care and pain/symptom management in hospice under physician supervision.
- Perform history and physical assessments and document findings.
- Develop individualized care plans and coordinate hospice services.
- Complete hospice face-to-face visits and assess eligibility.
- Communicate with interdisciplinary team to plan and adjust care.
- Educate patients and families and ensure safe home environments.
