Job summary

Dover Health is seeking a Nurse Practitioner to provide PRN hospice care with a patient-centered approach. The role conducts face-to-face visits to assess hospice eligibility, recertify care, and manage symptoms for terminally ill patients.

Responsibilities include collaborating with an interdisciplinary team, documenting care in EMR, and supporting end-of-life planning. Travel to patient homes and facilities is required, with an emphasis on compassionate, compliant care.

Qualifications

• Master’s degree in Nursing from an accredited program.
• Current RN and NP licensure in the state of practice; national NP certification.
• Minimum of 1–2 years of clinical experience in hospice, palliative care, geriatrics, oncology, or home health.
• CPR certification and, if applicable, controlled substance agreement.

Responsibilities

• Conduct face-to-face assessments for hospice eligibility and recertification.
• Develop and implement individualized care plans addressing physical, emotional, and spiritual needs.
• Manage pain and symptoms, including prescribing medications and adjusting treatments as needed.
• Collaborate with physicians, nurses, social workers, chaplains, and other healthcare professionals to provide comprehensive care.
• Provide emotional support and education to patients and families regarding disease progression, prognosis, and end-of-life decisions.
• Document patient care accurately in electronic medical records and ensure compliance with hospice regulations and Medicare standards.
• Serve as a clinical resource for staff and the community regarding hospice eligibility, levels of care, and symptom management.
• Travel to patient homes, hospice facilities, or long-term care centers as required.
• Other duties as assigned.
