Job summary

Brookhaven Hospice is currently hiring Per Diem dedicated Hospice Nurse Practitioners to deliver exceptional patient care in the Seacoast area of New Hampshire. You will evaluate health histories, diagnose conditions, and develop personalized care plans in collaboration with patients, families, and the care team.

The role involves palliation, coordination of care with the multidisciplinary team, patient and family education, and staying compliant with hospice regulations.

Qualifications

• Master's degree and 1 year hospice or palliative care preferred.
• Current unencumbered RN and NP licenses in the state of practice.
• CPR certification and computer proficiency including EMR.

Responsibilities

• Deliver comprehensive medical care and palliate terminal illnesses under physician supervision.
• Collaborate with the hospice medical director and attending physicians on care plans.
• Coordinate patient care with the inter-disciplinary team to ensure holistic support.
• Educate patients and families on care goals, prevention, and spiritual concerns.
• Provide input on eligibility for initial certification and recertification.
• Maintain up-to-date knowledge of hospice regulations and compliance.
