Job summary

Crescent Hospice is actively seeking Per Diem Nurse Practitioners to deliver compassionate hospice care around Greenville. You will evaluate health histories, diagnose conditions, and develop individualized care plans in collaboration with patients, families, and the hospice team.

Responsibilities include coordinating care with the interdisciplinary team, educating patients and families, and ensuring compliance with hospice regulations.

Qualifications

• Master's degree in nursing or related field; hospice/palliative care experience preferred.
• Excellent verbal and written communication abilities.
• CPR certification may be required per state.

Responsibilities

• Deliver compassionate medical care, palliate, and manage terminal illnesses under supervision of a physician.
• Collaborate with attending physician, hospice medical director, and other practitioners for eligibility and care planning.
• Coordinate patient care with the interdisciplinary team to ensure a patient-centric approach.
• Educate patients and families, addressing preventative care, psychosocial, and spiritual needs.
• Provide input on initial certification and recertification decisions for hospice care.
• Stay compliant with Medicare/Medicaid, state, and federal hospice regulations.
