Job summary

Crescent Hospice is actively seeking Per Diem Nurse Practitioners to deliver compassionate hospice care around Sumter. You will evaluate health histories, diagnose and manage terminal illnesses, and develop personalized care plans under physician supervision.

The position is per diem with a focus on compassionate, patient-centered care. Benefits include health coverage, tuition assistance, PTO and retirement plans, with onboarding to support a smooth transition.

Qualifications

• Master's degree with NP licensure preferred; hospice/palliative care experience is a plus.
• Current unencumbered RN and NP licenses in state of practice.
• CPR certification and valid driver’s license required.

Responsibilities

• Provide comprehensive hospice medical care under physician supervision.
• Collaborate with hospice team to assess eligibility for hospice and ongoing care needs.
• Coordinate patient care with interdisciplinary team for holistic support.
• Educate patients and families on care goals, preventive care, and support services.
• Advise Hospice Medical Director on eligibility for initial certification and recertification.
