Job summary

Crescent Hospice is currently hiring Per Diem dedicated Hospice Nurse Practitioners to deliver exceptional patient care in and around the Florence Area. The role includes evaluating health histories, diagnosing conditions, and developing personalized care plans in collaboration with patients and families.

The position emphasizes compassionate, holistic hospice care with supervision by a collaborating physician, plus a strong onboarding program and competitive benefits package.

Qualifications

• Master's in Nursing preferred
• Current SC NP license required
• CPR certification preferred
• Hospice experience advantageous

Responsibilities

• Deliver compassionate medical care for hospice patients under physician supervision
• Collaborate with hospice team to assess patient eligibility for care
• Coordinate and oversee patient care with interdisciplinary team
• Educate patients and families on care goals and options
• Support certification decisions for hospice eligibility and recertification
• Maintain knowledge of Medicare/Medicaid/state/federal hospice rules and regulations
