Job summary

Amedisys, part of the Optum family, seeks a Per Diem Hospice Nurse Practitioner to deliver compassionate, patient-centered end‑of‑life care in home-based settings across Georgia.

Responsibilities include comprehensive assessments, symptom management, and collaboration with an interdisciplinary hospice team to support patients and families at the end of life. A valid NP license and Medicare billing privileges are required.

Qualifications

• Current CPR certification is required.
• Nurse Practitioner license in the state of assignment required.
• Must have Medicare billing privileges as a nurse practitioner.
• Must meet physician-nurse practitioner restrictive collaborative requirements per state.
• Insurance as required by state law for NP practice or group coverage with employer.

Responsibilities

• Communicate significant findings to the clinical manager/attending physician and interdisciplinary team.
• Complete face-to-face encounters and assess disease changes, functional decline, and quality of life.
• Order and review labs, imaging, and other diagnostics for palliation.
• Make referrals to other disciplines as indicated by patient needs.
• Participate actively in interdisciplinary team conferences.
