Job summary

Elite Hospice, part of LHC Group and a member of the Optum family, is seeking a Per Diem Hospice Nurse Practitioner to provide palliative care, symptom management, and supportive services to hospice patients per physician referral.

Responsibilities include face-to-face visits within time limits, coordinating plans of care, and collaborating with an interdisciplinary team to support patients and families. A strong bedside manner and field readiness are essential.

Qualifications

• Active, unrestricted Nurse Practitioner license in the state of practice.
• Current certification as a Clinical Nurse Specialist or Nurse Practitioner.
• Current certification by ANCC, NBCHPN, or AANP.
• Current driver's license and access to a reliable vehicle.
• 5+ years nursing experience.

Responsibilities

• Makes face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and reports clinical findings to the Hospice Medical Director.
• Consults with the Hospice Medical Director as needed, informs primary physician of recommended services, and collaborates with other physicians as needed.
• Initiates and coordinates the plan of care. Documents problems, goals, interventions, and patient/family response to hospice care. Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care.
• Instructs and supervises the patient/family in self-care techniques when appropriate.
• Maintains accurate and relevant clinical notes regarding the patient's condition.
