Job summary

Crossroads Hospice is actively hiring Per Diem dedicated Hospice Nurse Practitioners to deliver exceptional patient care in the Memphis area. You will provide comprehensive hospice medical care, develop personalized care plans, and collaborate with patients, families, and the care team to ensure holistic support.

The role requires a Master’s in Nursing (NP) with hospice experience preferred, active RN/NP licenses, CPR certification, and a valid driver’s license.

Qualifications

• Master's degree in nursing; NP with hospice/palliative care experience preferred.
• Current state RN and NP licenses in good standing.
• CPR certification and valid driver's license are required.

Responsibilities

• Deliver comprehensive hospice medical care and palliate terminal illnesses under physician supervision.
• Collaborate with attending physician, hospice medical director, and other providers for patient eligibility and ongoing care needs.
• Coordinate patient care with the interdisciplinary team to ensure a holistic, patient-centered approach.
• Educate patients and families on care options, preventative care, and psychosocial concerns.
• Provide input on initial certification and recertification decisions for hospice care.
• Stay compliant with Medicare/Medicaid, state, and federal hospice regulations.
