Job summary

Qualifications
- Active Nurse Practitioner license in Tennessee with prescriptive authority.
- Board Certification in Adult-Gerontology, Family Practice, or related specialty.
- At least 1 year of hospice or palliative care experience.
- Strong documentation and time management skills.
Responsibilities
- Conduct timely and compliant F2F hospice recertification visits in patient homes, skilled nursing facilities, or assisted living settings
- Document clinical findings in accordance with Medicare hospice regulations and Shalom standards
- Collaborate with the interdisciplinary group (IDG) to support eligibility determinations and ongoing care planning
- Maintain schedule flexibility based on visit volume and regulatory timeframes
