Crossroads Hospice is seeking Per Diem Nurse Practitioners to deliver compassionate hospice care in the Memphis metro area. You will evaluate health histories, diagnose conditions, and create personalized care plans in collaboration with physicians and the hospice team.

You’ll coordinate with caregivers and the interdisciplinary team to ensure holistic, patient-centered care that aligns with Medicare guidelines and state regulations.

Formación

• Master's degree in nursing or related field.
• Current unencumbered RN and NP licenses in the state of practice.
• Nurse Practitioner certification; CHPN preferred.

Responsabilidades

• Provide comprehensive hospice medical care and manage terminal illnesses under physician supervision.
• Collaborate with hospice medical director and other practitioners to assess eligibility and ongoing care needs.
• Coordinate patient care with the interdisciplinary team for holistic, patient-centered plans.
• Educate and counsel patients and families on care goals, preventive care, and psychosocial needs.
• Offer input to Hospice Medical Director on initial certification and recertification decisions.
• Maintain up-to-date knowledge of Medicare/Medicaid and state/federal hospice regulations.
