Job summary

Metro Physical & Aquatic Therapy is seeking a Per Diem Home Care Physical Therapist Assistant to provide in-home care under supervision of a licensed PT. The role covers Ocean County and offers flexible scheduling with part-time hours and competitive compensation of USD 50–55 per hour per visit.

Candidates must be a graduated Physical Therapist Assistant and licensed or eligible in New York; First Aid, AED and CPR certifications are required.

Qualifications

• Physical Therapist Assistant program graduate.
• Licensed PTA in NY or eligible.
• Mandatory certifications: First Aid, AED, CPR.
• Home health experience preferred but not required; mentor provided.
• Ability to travel to patients' homes within the geographic territory.

Responsibilities

• Accept and treat patients in designated geographic area.
• Provide PT treatments in patients' homes under supervision.
• Monitor and report changes to the supervising PT promptly.
• Communicate with patients, caregivers and the interdisciplinary team.
• Instruct patients and caregivers in exercises and safety measures.
• Update care plans based on patient progress with the PT.
• Document treatments and observations per standards.
• Participate in case conferences and discharge planning with the PT.
• Adhere to federal, state, and local regulations and policies.
