Job summary

Metro Physical & Aquatic Therapy is seeking a per diem Home Care Physical Therapist Assistant to provide in-home physical therapy services under the supervision of a licensed Physical Therapist. Flexible schedule and per diem compensation are offered, with a focus on patient-centered care.

You will treat patients in their homes within the Brooklyn, NY area, document treatments, communicate with the supervising PT and care teams, and participate in ongoing care planning with emphasis on safety

Qualifications

• PTA program graduate, eligible for NY PTA licensure.
• Active NY PTA license or eligibility required.
• Mandatory First Aid, AED and CPR certifications.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide physical therapy treatments to patients in their homes under the licensed PT's plan of care.
• Monitor and report changes in patient status to the supervising PT in a timely manner.
• Communicate effectively with patients, caregivers and interdisciplinary team members to ensure coordinated, effective care.
• Instruct patients and caregivers in exercises, safety awareness and use of assistive devices as directed by the supervising PT.
• Assist the PT in updating care plans based on patient progress.
• Document patient treatments and observations accurately and promptly per standards.
• Participate in case conferences and contribute to care planning and discharge in collaboration with the PT.
• Adhere to all regulations and agency policies and professional standards of practice.
• Ability to travel to patients’ homes within the assigned geographic territory.

Education

Physical Therapist Assistant Program Graduate

Licensed Physical Therapist Assistant in New York or eligible

First Aid, AED, CPR certifications

• Compensation: USD 45 - USD 55 - hourly

• Physical Therapist Assistant Program Graduate
• Licensed Physical Therapist Assistant in the state of New York or eligible.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
