Metro Physical & Aquatic Therapy is seeking a Per Diem Home Care Physical Therapist Assistant to provide in-home care under supervision of a licensed PT. The role covers Ocean County and offers flexible scheduling with part-time hours and competitive compensation of USD 50–55 per hour per visit.

Candidates must be a graduated Physical Therapist Assistant and licensed or eligible in New York; First Aid, AED and CPR certifications are required.

Competenze

• Physical Therapist Assistant program graduate.
• Licensed PTA in NY or eligible.
• Mandatory certifications: First Aid, AED, CPR.
• Home health experience preferred but not required; mentor provided.
• Ability to travel to patients' homes within the geographic territory.

Responsibilities:

• Accept and treat patients within your designated geographic area


• Provide physical therapy treatments to patients in their homes, as outlined in the licensed PT’s plan of care


• Monitor and report changes in patient status to the supervising PT in a timely manner


• Communicate effectively with patients, caregivers, and interdisciplinary team members to ensure coordinated, effective care


• Instruct patients, family members, caregivers, and Home Health Aides in exercises, safety awareness, and use of assistive devices as directed by the supervising PT


• Assist the PT in updating care plans based on patient progress and response to treatment


• Document patient treatments and observations accurately and promptly, in accordance with agency and regulatory standards


• Participate in case conferences and contribute to the care planning and discharge process, in collaboration with the PT


• Adhere to all federal, state, and local regulations as well as agency policies and professional standards of practice
