Job summary

Metro Physical & Aquatic Therapy seeks a motivated Per Diem Physical Therapist Assistant for in-home care in Ocean County, New Jersey. You will work under a licensed PT, delivering home-based therapy and coordinating with families and staff to ensure quality care.

Ideal candidates are PTA graduates, NY-eligible, and certified in First Aid, AED, and CPR. Flexible scheduling and full W-2/1099 options are offered to fit your workflow.

Qualifications

• Licensed Physical Therapist Assistant (NY) or eligible.
• PTA program graduate.
• Mandatory certifications: First Aid, AED, CPR.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide physical therapy treatments in patients' homes under supervision of a licensed PT.
• Monitor and report changes in patient status to the supervising PT promptly.
• Communicate with patients, caregivers and the interdisciplinary team for coordinated care.
• Instruct patients and caregivers in exercises, safety and use of assistive devices as directed by the PT.
• Assist the PT in updating care plans based on patient progress.

Home Care Physical Therapist Assistant- Per Diem (Ocean County)

• Part-time


• Compensation: USD 50 - USD 55 - hourly

Responsibilities:

• Accept and treat patients within your designated geographic area


• Provide physical therapy treatments to patients in their homes, as outlined in the licensed PT’s plan of care


• Monitor and report changes in patient status to the supervising PT in a timely manner


• Communicate effectively with patients, caregivers, and interdisciplinary team members to ensure coordinated, effective care


• Instruct patients, family members, caregivers, and Home Health Aides in exercises, safety awareness, and use of assistive devices as directed by the supervising PT


• Assist the PT in updating care plans based on patient progress and response to treatment


• Document patient treatments and observations accurately and promptly, in accordance with agency and regulatory standards


• Participate in case conferences and contribute to the care planning and discharge process, in collaboration with the PT


• Adhere to all federal, state, and local regulations as well as agency policies and professional standards of practice

  • Physical Therapist Assistant Program Graduate
  
  
  • Licensed Physical Therapist Assistant in the state of New York or eligible.
  
  
  • Mandatory Certifications: First Aid, AED, & CPR Certifications.
  
  
  • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
  
  
  • Ability to travel to patients’ homes within the assigned geographic territory
